Every service is guided by an individual's Person-Centered Support Plan and centered on meaningful participation at home, at work, and in the community.

Personal Support & Assistance
Individualized assistance with daily routines and activities that help people live more independently.
Supports may include assistance with household activities, personal routines, transportation, community participation, skill development, and other needs identified in the person's support plan.

Community Integrated & Day Services
Building independence, social relationships, and community engagement through local events, volunteering, life-skills training, and hobbies.
Time is spent in the community rather than in a facility, with activities chosen by the person taking part.

Supported Employment
Helping neighbors succeed in integrated community workplaces with job coaching and transportation support.
Coaching adapts as confidence grows, and we work with employers so the fit lasts beyond the first few weeks.

Residential & Living Supports
Flexible residential options, including Supported Living, Family Model care, and Semi-Independent Supports, designed to provide safe, comfortable, self-directed home environments.
Each option is a real home, chosen with the person, where they set the rhythm of the household.

How Services Are Funded
Neighbor Links works with individuals receiving services through Tennessee's home and community-based programs, including DDA and TennCare programs such as ECF CHOICES and CHOICES.
Eligibility, service authorization, and funding are determined by the applicable program and the individual's support plan.
